Your First Visit
Your new patient experience starts the moment you contact our office to schedule an appointment. After confirming a date and time for your visit (approximately within one week of your initial call), we’ll prepare to welcome you to our office. On the day of your arrival, you will be greeted by our warm and welcoming front desk staff before completing any necessary paperwork (unless you previously filled out this information beforehand). You’ll then receive a complimentary gift bag.
One of our kind and caring dental assistants will then escort you to a treatment room to review your paperwork, discuss any concerns, take the necessary X-rays, and perform a wellness scan using our iTero digital scanner. You’ll then meet Dr. Howard Brooks or Dr. Stephanie Brooks who will complete your exam and discuss any findings with you. After answering your questions, you’ll be scheduled for your next visit, which may include a cleaning or any other recommended form of treatment.

As nice as our waiting room is, we know that you probably want to spend as little time in it as possible! To speed up your check-in process, simply click on the link below to download our forms and fill them out before your first visit. When the day of your appointment arrives, simply bring the completed forms with you, and we’ll get you right in to see your dentist! If you have any issues accessing our forms, please let us know.

At Brooks Dental, our team is here to make it easy for you to use your insurance benefits. We’ll do a complimentary benefits check so you know exactly what is and isn’t covered by your plan, and we’ll even file the claim on your behalf.
